If you expect us to accept your insurance payment for the office visit, please bring your insurance card. Also confirm that we do participate, before your appointment, as you are financially responsible for the fee if we are not on your insurance panel.

Co-payments are due at the time of the visit for insurances that have a fixed co-payment. For Medicare you will be billed for your deductible and co-payment when we receive the "Explanation of Benefits" from Medicare.

We accept Visa, MasterCard and AMEX for co-payments or outstanding balances.

Please note that you are welcome to use our services even if is not covered by your insurance but you will be responsible for payment of fees at the time of the visit.
